A new case series published in the Journal of Oral Implantology examines techniques for preventing and managing dental implant displacement in areas of weakened jawbone. Researchers highlight the use of bone grafting, careful implant selection, and surgical planning to improve implant stability and support successful treatment outcomes.
GLEN ALLEN, Va., Oct. 21, 2026 /PRNewswire-糖心Vlog/ -- Journal of Oral Implantology 鈥� Dental implants placed in the back of the lower jaw generally have predictable outcomes, but areas of unusually low bone density can create challenges. In focal osteoporotic sites (small areas of bone with more marrow than solid bone), weakened bone may make it difficult for an implant to remain stable and, in some cases, cause it to shift deeper into the jaw during placement.
Researchers from Smile USA Educational Excellence in Elizabeth, New Jersey, Rutgers School of Dental Medicine in Newark, New Jersey, Roseman University in Utah, Jacksonville University in Florida, the All India Institute of Medical Science in New Delhi, India, and the Ahmedabad Dental College & Hospital in Gujarat, India, recently published a clinical case series in the Journal of Oral Implantology examining ways to manage and prevent this weakened jawbone complication. Through these cases, Shankar Iyer, DDS, MDS, and colleagues "document the management of apical displacement of implants through cases 1 and 2 and suggest surgical protocols, including the osteotomy grafting technique to prevent implant displacement, through cases 3 and 4."
In 2 cases, the researchers used particulate allogeneic bone graft to fill the prepared implant site. Compacting the graft into the site created more support for the implant and encouraged new bone formation. Depending on the amount of cortical bone (outer bone layer) present, the implant could be placed immediately or after the grafted area had time to heal. In one case, an implant placed immediately into the grafted site remained functional for 5 years, with no adverse findings reported during follow-up.
These four cases highlight several areas that may help clinicians reduce implant displacement risk. These include carefully evaluating cone-beam computed tomography (CBCT) scans, paying attention to resistance while drilling, adjusting implant size as dictated by site anatomy, selecting an implant design that provides stability near the base of the bone, and avoiding placement of the implant too far beyond that base point.
Iyer and colleagues conclude that "These suggestions are recommended for both seasoned experts and newly trained practitioners. Osteotomy grafts using allogeneic bone graft benefit clinicians by providing a positive tissue bed created by the graft material. The characteristics of the graft further contribute to a successful treatment outcome."
